Abstract

A young 33 year old male presented with complain of discomfort in his eye. No other complains. Patient had borderline high intra ocular pressure of 22 mm Hg. Fundus and disc examination revealed suspected glaucomatous disc. Patient had been misdiagnosed as glaucoma, as allergic conjunctivitis, malingering in few different places. He had 6/6 vision in both eye and so not examined properly since he presented during second wave of covid time.
Borderline high IOP with disc changes took glaucoma as 1st differential. Other differential of neuro ophthalmological diseases took a back seat. Patient been advised visual field, RNFL OCT, CCT as baseline test but due to money constraints he could not able to do those tests. With fear of Covid, patient been just examined superficially until a sudden observation of Right side head tilt noticed. There is limitation of movement and abnormal eye movement. CEMRI brain advised
MRI suggestive of neoplastic lesion. ?Astrocytoma ? granulomatous lesion.
