Abstract

Incidence of severe vision loss is reported to be less than 0.08% due to the complication of Acute Post operative Endophthalmitis in routine Cataract surgery.
Our patient 53y/M came for routine cataract surgery (Vision 6/24 NS2 Grade with hypertension and alcohol abuse) which was uneventful with Phacoemulsification & IOL with 6/6 vision on Day 3. Patient came with HM+ and congestion on Day 7, operated the next day with PPV 23G and Intravitreal Antibiotics with Steroid, after baseline investigations and monitoring (SL photography, USG). He improved to 6/9 in a month with clear media, healthy retina & optic disc (ONH). Sadly 4 months later presented with FC1m but this time with mild nasal blurring of ONH & clear media. He was evaluated with Neurology (MRI, MRA, MRV, lumbar puncture & CSF analysis).
We suspect a compromise to the vascular supply of the ONH with multiple stresses of Endophthalmitis, PPV, alcohol abuse & irregular hypertension control leading to atypical NAION with SVL.
