VT0314 : Strategies in the management of posterior capsular rupture in cataract surgery

Abstract

This video shows steps of PCR management
Immediately after recognizing PCR , do not panic, keep calm and follow these basic rules
1. Inject viscoelastic through the side-port, before removing phaco hand piece [irrigation mode] from the eye to maintain anterior chamber, minimize the vitreous prolapse and stabilise the remaining lens material
2. Perform a good automated anterior vitrectomy, remove nucleus matter and then cortex, ensuring aspiration of the cortex towards the tear and not away from it. Dry aspiration is most suitable
3. High cut rate and low vacuum to prevent extension of the tear, hydration of the vitreous, and flushing of the vitreous
4. Complete vitreous removal is indicated by a round pupil, the falling back of the iris, and the formation of a single air bubble after air injection
5. Depending on the amount of capsular support available –
a. PCIOL in the bag or sulcus-fixated IOL can be implanted
b. If support is inadequate, iris or sclerally fixated IOL is implanted
