Abstract
A 40 year old male with high myopia presented with decreased vision in left eye. Best corrected visual acuity in left eye was 20/200 with -11DS with axial length of 28.61mm. He underwent uneventful left eye phacoemulsification for grade III nuclear sclerosis. However a large air bubble was observed coming from nasal side behind the posterior capsule during cortical matter aspiration. Posterior capsule was intact and there was no evidence of zonular dehiscence peroperatively. The bubble was displaced towards periphery by manipulating the capsular bag and a three piece intraocular lens was implanted in the bag. On the first postoperative day, there was no air bubble and patient had a visual acuity of of 20/30. Patients with high myopia can develop entry of air through normal zonules to vitreous cavity during phacoemulsification which can be called as pneumophacocele which resolves automatically.
