FP2095 : A Rare case of Idiopathic orbital inflammatory syndrome with late response to steroid treatment.

Abstract

A 54-year-old man presented with swelling and pain of the right eye that was associated with proptosis, ptosis and restriction of eye movements in all gazes which was progressive since 2 months. A differential diagnosis of orbital cellulitis, thyroid orbitopathy, orbital neoplasm, lymphoproliferative disorder and idiopathic orbital inflammatory syndrome (IOIS) was considered. Relevant laboratory investigations were performed, which showed elevated CRP and ESR. Magnetic resonance imaging was suggestive of IOIS. The patient was started on IV Steroids (Methyl Prednisolone) with IV antibiotics and T. methylcobalamine, but did not show clinical improvement. Patient was discharged with oral steroids. Patient was called for follow up weekly. Patient started showing improvement 2 weeks after discharge. Patient was followed up regularly. After 6 weeks of oral treatment patient showed symptomatic improvement in EOM, reduction in proptosis and pain. Patient is currently in remission.
