Abstract
Aims To identify risk factors for local recurrence and metastasis in sebaceous gland carcinoma (SGC) of the eyelid. Methods: Retrospective review of 183 consecutive patients, with histopathological diagnosis of SGC. Results: Mean age was 57 ± 13 years. Incidence rate of local recurrence and metastasis was 0.12 and 0.04 per person per year respectively. Recurrence was higher in those with prior incisional biopsies, longer duration of symptoms, lymphovascular invasion (LVI) and pagetoid spread (PS) away from the tumour. Metastasis was associated with caruncular lesions, size more than 20mm, PS and LVI. Multivariate analysis showed independent risk factors for recurrence to be LVI (OR 3.2, 95% CI 0.9 – 10.7) and PS (OR 1.5, 95% CI 0.5 – 4.1), and that for metastasis to be size more than 20mm (OR 3.3, 95% CI 0.82- 13.79). Conclusions: Risk factors for local tumour recurrence and metastasis of SGC were identified. Standardised surgical approach could be pivotal in optimising outcome.
