FP2634 : INJECTING HOPE – Intralesional Retrobulbar Voriconazole Injection for Orbital Fungal Granuloma

Abstract

A 52 year old gentleman presented to us with protrusion and diminution of vision in his Right Eye since 2 months.
His Right Eye had a vision of Counting Fingers at 1meter with ocular motility restriction (depression, elevation and adduction).
Left eye vision 20/20 and ocular motility was normal.
CT scan of orbit revealed a Right Sino-orbital mass eroding medial superior and inferior orbital wall. Previous reports of endonasal biopsy done elsewhere was inconclusive(chronic inflammation).
The patient underwent incision biopsy from epicenter of lesion via trans-caruncular approach . Histopathology and microbiology reports suggested a fungal granuloma (Aspergillus Fumigatus).
The patient was treated with 3 retrobulbar injections of Voriconazole along with the ongoing Oral Voriconazole.
His vision improved from CF1m to 20/20p in the Right Eye, regained full ocular motility with almost complete resolution of the lesion in the recent CT Orbit scan. Thus our patient saw his road to recovery.
