Abstract

Purpose: To present a rare case of fulminant bleb associated endophthalmitis with atypical etiology.
Case: A 16-year-old male presented with sudden, progressive, painful diminution of vision for 1 day in right eye. He was a known case of primary congenital glaucoma and had undergone both eye trabeculectomy with mitomycin 2 years back. On examining; right eye vision acuity was perception of light with accurate projection of rays, revealed thin necrotic bleb (3-4 clock hours) with pus points, surrounding conjunctival congestion, associated with hypopyon in anterior chamber and yellow glow on retro illumination. Membrane like echoes on ultrasound B scan was noticed. Left eye also has thin, cystic diffuse bleb. Patient underwent PPV along with intravitreal antibiotics. The culture was revealed Neisseria Mucosa and was sensitive to piperacillin. Piperacillin was administered intravenously and subconjunctivally. At 2 months follow-up VA was 6/18 and media grade 1 with attached retina.
