FP2059 : Topical capsofungin for management of refractory fungal corneal ulcers

Abstract

Study Design – Case study
Purpose – To report two cases of recalcitrant fungal keratitis managed with topical capsofungin acetate.
Methods – A 68 years old female and 50 years old male presented with non healing fungal corneal ulcer. Both patients had positive KOH smear with filamentous fungi. They failed to respond to conventional treatment along with intrastromal voriconazole. Topical Capsofungin eye drops 0.5% were given in hourly dose. The other antifungals were stopped. After one week of therapy, the patients started responding. Total healing took four weeks in female patient and six weeks in male patient.
Discussion – Treatment of recalcitrant fungal keratitis can be challenging at times. The antifungal drugs have the issue of poor penetration in cornea. In our two such patients we tried capsofungin eye drops hourly in first week, followed by gradual tapering as per the response.
Conclusion – Topical Capsofungin may be used as an alternative for refractory corneal ulcers.
