FP1155 : CLOFAZIMINE INDUCED OCULAR PIGMENTATION : A RARE CASE

Abstract

INTRODUCTION:Systemic medications gain access to cornea & conjunctiva via tear film,aqueous,limbal & conjunctival vessels.Corneal changes are dose related,with reduced visual acuity, photophobia;& may reflect risk of lenticular or retinal changes.Clofazimine is a lipophilic iminophenazine antibiotic used in the treatment of leprosy

HISTORY:51yr old lady with borderline lepromatous leprosy came with brownish-red discoloration of conjunctiva x1 month.Last 5 years,she was on Clofazimine 300mg monthly pulse & 50mg daily dose,Rifampicin & Dapsone.Skin changes recorded 2 months after starting therapy,no ocular changes

EXAMINATION:BCVA(OU) 6/6,N6.Anterior segment exam,fine,brownish-red crystalline deposits in the superficial layers of both cornea and conjunctiva.IOP & fundus normal.

TREATMENT:Clofazimine dose tapered in 4 months-given 100 mg alternate days x 2 months;then 300mg monthly x 2 months,then stopped.She was on follow up 6 monthly.Pigmentation became less dense on follow up
