VT0556 : The Macular Hole Plug

Abstract

Purpose: To show retinal graft technique in a case of full thickness macular hole
Case: A 18y/M presented with a large macular hole (base diameter 1802 microns). 3 PPV ports made. PVD was induced with the use of IVTA. 4th port was made and bimanual surgery was done using inferior chandelier. Retinal graft site was marked by 3 layers of EL in STQ and then graft harvested with horizontal and vertical intraocular scissors. Under PFCL, graft was transferred to macula using ILM and Picks forceps. The graft was trimmed to approximately 10% larger than the actual size of macular hole. After securing the graft, the harvest and grafting site were rechecked and C3F8 gas injected
On POD 1, there was closure of macular hole noted on fundus examination.
On follow-up at 3 weeks, VA was 6/60p and OCT showed complete closure of macular hole with graft.
Conclusion: Retinal grafting can be attempted by this technique to achieve anatomical closure and prevent further loss of vision.
