VT0237 : PPV in ocular trauma akin to opening Pandora’s box- meticulous repair key to successful outcome

Abstract

Blunt ocular trauma leads to multiple vision-threatening complications involving the retina and surrounding structures. This surgical video demonstrates a plethora of posterior segment manifestations of blunt ocular trauma in a 55-year-old male who sustained left eye (OS) injury. On examination, the patient had lamellar corneal laceration, posteriorly dislocated lens, iridodialysis, & VH. A standard 3-port PPV was performed. Core vitrectomy was initiated to clear the VH after which a dislocated lens was noted in the mid vitreous cavity. PPL was performed using the 23g cutter. To our surprise, we noted a traumatic macular hole (MH) along with macula involving temporal RRD, and a series of circumferential retinal breaks in temporal and superior retina. MH repair with inverted ILM flap technique and RRD repair were done. Post-operative examination showed an attached retina and a closed macular hole. Meticulous repair and patience are the keys to successful outcomes in such cases
