Abstract

A 48 year old male presented with advanced Fusarium keratitis. He had total corneal ulcer with dense anterior chamber exudates. He underwent large diameter therapeutic penetrating keratoplasty. Post operative course was rough with occurrence of vitritis and graft inflammation. The patient underwent 6 intravitreal injections. Subsequently the patient underwent a re-graft with cataract extraction with vitrectomy. The surgery posed a unique challenge due to presence of a dense thick retro corneal membrane, iris adhesions and dense cataract. Histopathological study of the excised cornea and retro corneal membrane showed plenty of inflammatory cells with eventually no evidence of infection. After multiple interventions, the globe could be salvaged and the patient is planned for future visual rehabilitation.
