Study design:case report
Purpose : we propose an atoll sign in a rare case of bilateral posterior lenticonus,considered positive in advanced cases of posterior lenticonus with intact posterior capsule.
Methods: Here,a 22 year old male presented with gradual diminution of vision in both eye since childhood and had aggravated since last 6 months.His BCVA was 5/60 in RE and 2/60 in LE.We found posterior lenticonus with cataractous changes in RE and advanced cataractous change with collapsed cone in LE.Patient was planned for phacoemulsification in right eye with PCIOL followed by left eye with lens placement in sulcus.
Results: Vision in RE improved to 6/6 but in LE didn't improve beyond 6/24.
Conclusion:we are proposing an atoll sign in advanced case of posterior lenticonus with intact posterior capsule in regard to its resemblance to water body surrounded by rocks.Similarly like atoll ,it appears like clear central posterior cone surrounded by cataractous lens material.
