FP2308 : TB or sarcoid? A perplexing case of recurrent granulomatous Pan uveitis.

Abstract

Study design – Case report
Purpose
To discuss a case of granulomatous pan uveitis treated as Tubercular uveitis in past unfolding into sarcoid uveitis in the treatment course over 5 years.
Methods
21Y/ female on ATT for abdominal tuberculosis. With bilateral disc edema and multiple choroidal granulomas, (hypo fluorescent both in early and late frames on FFA and hypocyanascence on ICGA). PET scan s/o pancreatitis, with enlarged retroperitoneal lymph nodes. HPE showed granulomatous inflammation, with negative staining for Acid Fast Bacilli. HRCT chest and serum ACE levels- WNL
Results:
Treated with oral steroids, azathioprine and ATT. Five months after stopping azathioprine, developed bilateral DOV with left preauricular swelling.
CEMRI-enlarged left parotid and submandibular glands. FNAC-numerous epithelioid cell granulomas with multinucleated giant cells and lymphocytes. ZN stain for AFB-negative
Conclusion: Tuberculosis could occur coincidently or in association with Sarcoidosis
